The final deadline for grant requests for both Regular and Summer Programs is January 15, 2005.

Any organization that did not submit a request for the deadline of September 15, 2004 or that is not in the midst of a multi-year grant is eligible to apply.


REGULAR PROGRAMS

A complete application includes the following (Maximum of 8 pages, minimum font size of 10. Please number pages):

  • Proposal cover sheet (enclosed)

  • The agency:
    • Brief history
    • Statement of mission
    • Description of current programs, activities and accomplishments, with highlights of the past year
    • Overall plans for the coming year

  • The project (funding request):
    • Project goals and objectives
    • How the project fits into your overall organizational mission
    • Constituency served/target population
    • How the program operates, including events and activities planned, days and hours of operation
    • Design of curriculum, if applicable
    • Key staff/volunteers involved and their roles (Indicate any new staff/volunteers required.)
    • Timetable for implementation
    • Interaction with other organizations
      (if relevant)
    • Budget, with line items
    • Funding sources including funds received, approved or pending.)

  • Evaluation. Please describe:
    • Expected results - what you expect to accomplish in the funding period
    • How you will define the project's success and how success will be measured
    • Anticipated consequences of eliminating or not undertaking the project
ATTACHMENTS
A complete application also includes the following attachments:
  • Board of Directors
    • Addresses and occupations
    • Indicate minority, low-income, gender, consumer and/or neighborhood representatives.
  • Finances of the organization
    • Current annual operating budget
    • Recent audited financial statement
    • Past major contributors, with amounts
    • Revenue sources including funds received, approved or pending
    • Recent applications with results
  • Personnel - attach resumes of top staff as well as any relevant key staff/volunteers involved in the specific project

  • Annual Report (if available)

  • A few examples of current, relevant newspaper or magazine articles about the organization's program, if available

  • A copy of the most recent IRS letter giving proof of the organization's tax-exempt status
